Job Title: Assistant Director - Supported Community Living

Employer: North Star Community Services

Location: New Hampton

Brief Job Description/Summary: Provide service coordination, individualized planning, and staff supervision to persons served, and oversees assigned Supported Community Living Office.

List of Job Duties:

-Oversee programs and services of assigned office

-Employ, train and supervise a sufficient number of staff members to provide efficient and effective services

-Facilitate, develop, implement and coordinate person-centered plans, case notes, and outcomes meet regulatory agency expectations

-Maintain records of persons receiving services according to agency policies and procedures and regulatory requirements

-Assist with budget preparations and ongoing financial monitoring

-Develop and maintain positive relationships with families, caregivers, referral sources, funding sources and other providers

-Perform and document all required safety drills

-Assign, coordinate and maintain company vehicles assigned to the program

-Ensure the office is maintained and stocked with supplies, coordinate lawncare, snow removal, etc.

-Report incidents and other safety issues to supervisor and inform the Director of any needs or concerns related to Supported Community Living

-Provide Medication Management and health related services and assist with direct services and personal care

Required Experience and/or Knowledge, Skills and Abilities: Participate as a member of a team, get along with others, and incorporate the core values of respect and dignity toward everyone. Qualifications include a BA in human service field and four years progressive experience working with persons with disabilities. Helpful qualifications: Crisis Prevention Intervention Certification & Medication Manager Certification. Applicant must meet requirements for background checks and driving record.
